Default create electronic sign in sheet

I am a professor, and I would like my students to sign-in on a lap-top. I
would like a form requesting the sign-in information for each individual,
once they complete it the information is added to a master list that only I
can view. I would prefer the information to end up in an exel workbook, but I
am open for suggestions. Anybody have any advice for me???
