Default Form behind cell entries

Good afternoon --

I would like to use Excel as a simple database, so that other users can fill
out forms. Rather than retyping and formatting the form in Excel (or
requiring them to print out a blank copy and reinsert it into the printer)
I'd like to paste a picture of the form, perhaps making it transparent, and
then writing code to fill the appropriate cells with the appropriate info.

So -- can I paste (for example) an IRS W-9 tax form into Excel, making the
blank parts transparent so I can align the boxes with Excel cells? I have
pdf's, but can convert into other formats if needed.
