Default Menu Option No Longer Valid

I had a .pdf-making program that left a menu option on my excel menu bar.
How do I get it removed permanently. I have already uninstalled the
software, but the menu items remain.

Hold the Alt key down and drag the menu item over the spreadsheet and let it go.

If it's only a single icon on a toolbar, I'd use:
Tools|Customize (just to see that dialog)
then drag that icon off the toolbar

Then close the dialog
