Default medications log templates

Any ideas for a "less extensive" medications log templates than that already
given? Just med name, dose, frequency, doctor, why taken. Simple. The one on
the MS Ofc. site is SO DETAILED it would be half-empty!!!! :-)

Default medications log templates

Try doing a Google search and see if you can find one without all the bells
and whistles. Other than that, it's roll-your-own.
