Default Adding a macro to a toolbar

I want to add a macro to a toolbar, which I have done before but now it does
not work so I must have forgotten something. I go to customise toolbars and
then select 'commands' and 'macros'. The problem is that my macros are not
shown, just 'Custom menu item' and 'Custom button'. How do I make my macros
visible?

Dudley,

Drag and drop the button to where you want it on the toolbar, right click
that button (before closing the Customize dialog box) and select "Assign
Macro..." at the bottom of the list.

How do you get to the "custom button" option? I can't find that anywhere? I'm
trying to add a custom button to my toolbar and assign a macro to it, but I
can't even find where to add the button?

Right click any toolbar
Select Customize
On the Commands tab select Macros from Categories
You'll see the custom button icon (smiley face) in Commands
Click it and drag it to your toolbar

When I right-click to assign macro, nothing happens. What am I doing wrong?

For additions to the worksheet menu bar, I really like the way John Walkenbach
does it in his menumaker workbook:

Here's how I do it when I want a toolbar:

In xl2007, those toolbars and menu modifications will show up under the addins.
