Default Diabetes tracker? BG, BP, Wt, INR, exercise, carbs - in 1 chart

I have seen excel BP trackers, weight trackers, BG trackers.
Does anyone know how to create a simple-to-use chart to track several items
at varying times of the day or week. For example:

BG will be monitored 1 to 4 x day,
Carbs - and other essential nutrients - 3 or more x day
BP, Wt, INR will be tracked randomly - weekly, monthly or more often as
required

Any assistance will be greatly appreciated. I hve very limited experience
with Excel

This depends on what you are tracking. If you are simply tracking the number
of times per day for BP, BG, etc (sorry, I do not know what these are) then
simply setup a small table with the column headers BP, BG, etc. For the row
headers enter the first date, e.g. 07/01/2007. Then use the fill handle
(tiny box at lower right corner of cell when selected) and drag it down the
rows. It will fill in sequential dates, starting with 7/1.

At the intersection of a date and column header (BP, BG) , enter the numbers
of times per day.

Do not put any blank rows or columns in the table. Select the table by
simply clicking inside it, and start the chart wizard. Try different charts
- stacked line charts, stacked bar charts, etc.
