Default Multiple Lookup with Criteria

Lookup criteria:

G2 = PROD A
H2 = MFG A
I2 = CASH
J2 = 10/15/2009

Maybe this array formula** :

=INDEX(E2:E6,MATCH(1,(A2:A6=G2)*(B2:B6=H2)*(C2:C6= I2)*(D2:D6J2),0))

** array formulas need to be entered using the key combination of
CTRL,SHIFT,ENTER (not just ENTER). Hold down both the CTRL key and the SHIFT
key then hit ENTER.
