Default how to place a decimal after 3rd digit

Thank you!! It worked. I did have to create the formula in a separate cell.
Once I got the numbers with correct decimals in place, I just copied and
pasted them into the original cells.

How do I place a decimal after the third digit of a number (e.g., ICD-9

I need to place it third from the beginning of the number.

You will need to enter the codes as TEXT. If you enter them as numbers, Excel
will drop the leading zeros and I know of no way to differentiate, for example,

012.8 from 128.0 if they are both entered as numbers without a decimal point.

Excel will store both as 128, but one is a type of TB, and the other is a
helminthiasis.

After entering the value as text, use:

=LEFT(A1,3)&"."&MID(A1,4,20)

20 is just some value longer than the longest code.
