Default greater than, less than, and in between

You don't need the third condition (<=94% will cover everything
else...assuming C6 does have a value and not #N/A)
Try
=IF(C6100%,"HIGH",IF(C6=95%,"WARNING","GOOD"))

btw which values are giving you a problem...

as well as many others. I know it is just me and I am not thinking over the
formula clear enough or should I take my decimals out further.
