Default Open Form when file opened

How do I create a form to open when a file/spreadsheet is opened?
Example:

Dept Name Hours Vac Sick Total
adb Joe 24 8 8 40(sum of the
hours entry)

The form would just be the information above to key in.
when the file is opened I want the keyer to see the fields that need to be
keyed in. I don't care if she can see the spreadsheet or not while she is
keying in information.
also the ability to 'Add New Employee'
