Table in A1:E4, Achievement in A6, Priority in B6, formula in C6

I've maintained another excel sheet which has 3 colums by names
Results,
Priority and Scores.
When i enter Excellent and Medium in respective Colums Results and
Priority,
it should fetch the Scores from the above table as 6.
Please help
