=INDEX(F5:O5,,MATCH(A5,{"Mail","Phone","Email","FA X","Courier"},0))*LOOKUP(A5,{"Courier","Email","FA X","Mail","Phone"},{0.1,0.2,0.3,0.35,0.5})
The match list can be any order, LOOKUP needs the list in sequence.
