Default How do I put a delete button on excel that will clear the fields?
CTRL + a(twice in 2003) then hit the "delete" button.

Record a macro while doing this then assign to a button from the Forms Toolbar
or drag one up to a toolbar from ToolsCustomizeCommandsMacros
