Default Modify Quick Access Toolbars - icons

I'm afraid I don't know the answer to modifying icons, but in answer to your
2nd question:

I found that when I click on one of the buttons already in my toolbars
(below the ribbon) this option is no longer grayed out. Sadly, it still does
not appear to have a modify icon feature.
