One try ..

Source data as posted assumed representative, and running in A1 down,
with replacement numbers listed in B1 down

Put in C1:
=LEFT(A1,LEN(A1)-SEARCH(",",A1))&TEXT(B1,"00000")&RIGHT(A1,LEN(A1)-SEARCH(",",A1,SEARCH(",",A1)+1)+1)
Copy down
