Same as previous versions.
In top row have titles in separate cells for Name, Address, Phone etc.
Select a range you know will be large enough, say A1:G500 and name it
"DataBase"(no quotes)under InsertNameDefine.
Select a cell anywhere in that range and DataForm.
Start typing.
