Those are for 2 separate cells, aren't they?

If cell A= P then I would like cell B to automatically fill with NA and
turn
black.

"Turn black", do you mean you want the background fill color to be black? If
the default font color is also black then you won't be able to see NA.

Formula for "cell B":

=IF(A1="P","NA","")

To make that cell turn black:

Goto FormatConditional Formatting
Cell value is: equal to: NA
Click the Format button
Click the Patterns tab
Click the black color square
OK out

if cell A = 11 or below & cell B = 5 or higher then cell C = needs
instructions

=IF(AND(A1<=11,B1=5),"needs instructions")
