Default Question on using Ron de Bruin's Excel menu sample code

Hi all:

Trying here to use Ron's sample code for creating an Excel menu of macro
entries that would exist in a single AddIn. I downloaded his "For My
Add-in.xlam" and enabled it as an AddIn. It worked wonderfully. I then
tried to create my own code for creating a similar menu and have had nothing
but problems.

Questions:

1) What causes the small green arrow icon to appear on the menu bar when
the AddIn is instantiated? I can't find this example anywhere!

2) I have seven macros that need to be accessible from the menu. I simply
hard-coded the references since there have been seven items for this menu
for at least the last 10 or more years (the application is legacy from Lotus
1-2-3). They seem to add, and the CommandBar appears to be present, but I
cannot make it visible. Doing so generates an error.

3) How can I get the menu item (the green arrow icon) to appear like it
does in Ron's sample?

Any help will be greatly appreciated!
