Default Macro for adding new items to list

I have a list for AutoFiltering with the top left corner at A12. I'd
like to have a button that I can push that will bring up prompts to
fill out a new entry.

Also, I'd like the first column to be a drop down list of values in a
Range Name called "TYPE"
