Default Creating a database in Excel

Hi Excel Gurus,
I need to create a simple database in Excel (e.g. Name,
Company, Address, Phone and a drop-down box) for
a "sample request" form for about 8 employees to use.

I don't have Access, but I'm sure this can be done in
Excel.

Anyone feel like telling me how?
