Gord, you don't live in the US, do you? It's a form needed to complete
taxes here. It's sent out just about this time every year.

Thanks Barb from this Canuck

I should have, at this time of year, associated a W-2 with taxes.

Up here we have T-2, T-4 forms(among others)
