I have limited data:
2006 annual sales by customer
Dec. 2007 sales by customer
sales margin by customer

My boss would like a 5 year sales/future earnins forecast - I would really
appreciate some direction, or template to work with!

Easy. List all your customers in order with their sales data. then, you need
to hold meetings with the Account Managers in order to get their estimates
of sales per customer for the next five years. then HALVE it. (I speak from
long experience, particularly if a great part of the salesman's remuneration
is commission!!!). The margin is just Sales times your expected (or
calculated) margin %age.....
