How do i insert a yes/no facility that can be clicked on at the end of a
question? I've seen what I want in the templates but I can't modify to offer
multiple answers.

Explore data validation?

Select the range,
click Data Validation
Under Allow: List
Enter in Source: Yes, No
Click OK

Thanks Max, it worked just as you said. However, as soon as a click on
another box it disappears only coming back when I return to the box. How do I
keep it visable for all to see?

One way is use an input message ..
Re-select the DV range, click Data validation
Click on the Input Message tab
Under Input message, type say: Select response
Click OK
