Here is Jim Rech's answer to a similar question in another group:
Comboboxes, etc., are on the Developer tab of the Ribbon. The Developer tab is not visible by default. To make it appear click the Office menu (the circle in the top left of Excel) and then click Excel Options at the bottom right of this menu. Select the third item on the "Popular" section. In case Bernard has mis-understood the 'Form' to which you refer, go to the QAT (Quick Access Toolbar) select the arrow to the right and then more commands..., select All commands from the drop down and scroll to Form... add it to the QAT and voila.
