Default questionnaire in excel

I'm trying to create a questionnaire where there are 3 possible responses to
each question, each wtih a different value, but where the respondants can
only give one answer per question. I've set it up using the checkboxes from a
formula toolbar at this point. Would anyone be able to explain to me in
layman terms how to somehow group the boxes such that only 1 out of 3 can be
checked at a time (any second box checked automatically clears the first) ?

As well, is there any way to have a function (or other?) at the end that
verifies if all questions have been answered and/or to tell the person which
ones are remaining?
