I need to create a form/worksheet for inputting suvey ianswers that will be
dumped into one spreadsheet/data base (USING EXEL). Anyone have any
suggestions?

Hi Kristen,

One option would be to use Excel's built-in Data Form.

Type the survey response headings into (say) the first row of a worksheet.
With one of the header cells selected,:
Data | Form... | OK

form.

If this approach is satisfactory, you might also wish to look at John
Walkenbach Enhancd Data Form which is freely downloadable at:
