how to use the form in excel
 

I would like ask how to use the form to input record in the excel sheet
automatically when open the excel , and click the button in the form to
back to the excel sheet.

record a macro opening the form...

place the macro in the workbook open event..


I don't know how or if the form can be displayed on its own,

I am assuming you are talking about the form selection in the data menu
