form/subform with buttons
 

I'm trying to make a subform that is a popup from a button. I have the form
displaying nicely & it displays the related fields (id & date), but the
record won't add into the sub table. I know its probably b/c the fields are
not being sent as values to those fields in the subtable, but I don't know
how to get that done.
