13 Greater %: =IF($B$9$B$10,$B$10,$B$10)
14 Bilateral Hearing Loss: =(((B12*5)+B13)/6)/100

I get a #VALUE! error in B14. I'm not sure what other information you need.
The formula in B14 uses the lesser and greater numbers from two other
cells/formulas. Any help would be appreciated! Thanks!
