Converting entered data into a different output

OK this may be a really stupid question but here goes.
I have a spreadsheet for which I want to set up a form for inputing two parameters which will then automatically generate an output (I have attached to this message a simple example of what I mean). I am sure there is a way I can do this either in a macro or through some sort of LOOKUP function but I can't quite get my head round it. Any suggestions (other than where I should put my head!) would be welcome. Note: I am not really an advanced user of Excel but have done basic Macros and simple LOOKUPs before but this is out of my reach. Thanks folks |
Creating a data entry form in excel
This isn't a binary group, so attachments aren't permitted. If you upload
