Default with multiple option buttons on my form how do I isolate groups

I created a form with the option button feature from the forms toolbar. I
have them grouped for different questions selections,but it won't isolate for
each group it will only select one for the whole worksheet. I need them to be
able to do one selection for each question.

Are you using the groupbox for each set of buttons?

Here is an example
1.) (inserted option button) Approved
(inserted option button) Pending
(Inserted option button)Rejected

2) (Inserted option button)Received
(Inserted option button)In progress
(Inserted option button)Not Received
The issue is that I have these all over my worksheet but when someone goes
to select an option it will only select one for the whole worksheet, when I
need to have it select one for each question (group) throughout the worksheet.

You need to use the group box to make them independent of each group, it's
in the same forms toolbar, just grab a hold of it with the mouse and drag it
around the 3 option buttons, do the same for all of the groupings.
