Check Box
 

When I insert a Check Box onto a worksheet, then right-click the box and
select Format Control, I don't get the Format tab to appear. Only 4 tabs
show up - Size, Protection, Properties (object positioning), & Web. How do I
get the Format tab to appear?

You used a checkbox from the control toolbox toolbar.

Delete that one and add a checkbox from the Forms toolbar to get to the Control
tab.
