Dumbfounded in Denver

Looking up two values
 
I have a spreadsheet that needs to look up a numerical value first and then
go to the month that we are closing e.g. February and apply the value in the
cell that is on the row first looked up. I have tried to do a vlookup nested
with an hlookup but can not get the formula right to make it work for
anything other than January data. Thanks for your help.
