Word Document that feeds a spreadsheet
 

I want to take a Word document and have the fields on the form feed a
worksheet in an Excel Spreadsheet. Can anyone point me to an area of
instuctions to help me accomplish this? It seems like a simple task,
but I'm not very Word proficient, as I usually work with numbers.
