I'm a newbie, urgently help needed
 

I don't know if this question has been asked before, I'm a newbie.
I'm writing a form in an excel :
Name :
Gender :
Age :
Phone # :
Address :
.......
Well, just simple entry like that, i'm wondering is there anyway t
write that entry on a vb. so I could print it a very single form
either adding on my database wich is I wrote on an excel sheet.

Help me pleas

I want the form as a standalone program

well, lessay i have this kind of form 00000.jpg
When i click finish, it's goes to an excel file (00001.jpg) an existin
file, not a new one, and it recorded into my database (another file o
my pc) 00002.jp
