What formula will read 2 separate values and give me a 3rd value?

I have to create a formula that reads an agent name and date and gives me the
value for that particular day. The problem is that each day the value is
different and I can't manually enter the data because even if there is no
data for that day, I need to show the person was here and an entry was made
as to why.

I'm sure you know exactly what you're looking for, but your post is
rather ambiguous. Giving more detail would help. For instance:

Where should the agent name and date be read from (another worksheet?,
another workbook?)? How is your data laid out? What value should the
lookup give you? How will you show that the person was there? Where does
the entry as to why go?
