I am looking for an employee review/evaluation form or template that I can
incorporate into our yearly management review process. Anyone have something
like that?

I actually didn't see one in the MSFT Excel templates section. But a Google
search for

Excel Employee Evaluation template

came up with lots of likely hits, including these two
