Default restore title bar icons

I totally lost the title bar (the blue bar at the top). How do I restore
this so I can minimize and close the program with these icons again?

Maybe you're in Full Screen mode.

Can you still see the worksheet menu bar and the View Dropdown?

If yes, then try
View|click Full screen (it's a toggle)

=====
Or maybe the title bar is just off the viewable screen.
try this:
alt-space bar
then hit M (for Move)

then use the arrow keys on the keyboard to move the window.

When you're happy, hit the enter key to anchor it.

Click on View Full Screen. That should fix the problem
