Anyone that can help me create a excel file such as this

The end user will need to open it in excel (microsoft)....

Have a look at this example.....its PDF, but you can see at the
bottom, the table/cells are present allowing the end user to input the
data.

Looking for advice, someone needs me to make them a cue sheet ASAP!

Here is the sample I am talking about:

I do not use Excel enough to make this work, I am making a mess of it.

I would do it in WORD - A lot easier to have "CELLS" of different
widths.

Yes. I could do it in Excel, but it wouldn't look EXACTLY like that one, but
reasonably so.
