Supply Order Form?

Has anyone created a good Office Supply order they would share?
 
Has anyone created a good Office Supply order they would share? I would love
to have a good layout for business supply orders without reinventing the
wheel. I prefer Excel forms, but Word is good too. Thank you in advance for
any help.
