Default How to create address lable from excel data

Can some1 try to help me ?
I have a large client database in excel file.....it look like below.....

NO C_NAME CODE PHONE1 EMAIL ADDRESS1 ADDRESS2 ADDRESS3
1.
2.
3.

how can i use the following data for lable, where my lable can display like :


1. C.Name Code 2. C.Name Code 3.
C.Name Code
ADDR1 ADDR1
ADDR1
ADDR2 ADDR2
ADDR2
ADDR3 ADDR3
ADDR3
Email Email
Email
PHONE1 PHONE1
PHONE1

Hope some1 can help or show me to do it....just a simple and nice one
just normal client listing and output to lable in word or excel...Thanks
