Default Separate First, Last Names

I have a spreadsheet of thousands of names. There is a NAME field which
needs to be separated into Last, First and Title.

The names are formatted like: Doe , James E, MD
(the space after last name is intentional)

I separated the last name into a separate field but can't figure out how to
get the first name and initial into another field and the title (MD) into a
3rd field. The First Name and Middle Initial don't have to be separated and
can be in a First Name field together.
