how do I create a locked form to fill in the blanks
 
how do I create locked form for filling in like name, address, date, etc,
somewhat like a fax form

Hi,

If you mean you want to use the sheet as a form but only allow entry in
certain cells, then select the cells you want unlocked and choose Format,
Cells, Protection, and uncheck Locked. Then choose Tools, Protection,
Protect Spreadsheet add a password or not, and unchieck Select locked cells
(or not).
