questionnaire template
 

I need a template for the following:
Type: Online questionnaire for clients
Format: Questions are a mix of Yes/No or multiple choice (some with more
than one answer selected from a list)
Output: All answers carry a weighting and total points are shown on a
sidebar as the client completes the questionnaire

The points per question are derived from a control benchmark database (not
visible to the client)

This is not an Excel question since it must be online.

Try ASP or the like.
