User form Samples (to query data)
 

Hi,
Could you pls provide me a sample of how to query using a User Form.
I'm able to build a form to accept user feeds.
Details of any website with this info will also be helpfull

The idea is to pull up a record entered in the database, & to update
new/relative fields.
