Would like a Template to conduct process review for departments
 
Hoping to undertake a study on various departments key outputs in regard to
the procedures that currently deliver the output, the measures there in and
who does what when and in what format is the data captured.

You could go to the Template Gallery and do a search on "review" or similar.

It sounds like you could be well served with just a simple database created
by placing your column titles across row A, and then filling in your data
points in the rows below, one row for each observation. Then the Autofilter
could be employed to separate like data into groups for analysis.
