Hi, I have a questionnaire that I want users to either select the response in
Column A, B, C, or D.

How do I set up a checkmark, that a user can click in the box to select one
of the columns as his/her answer.

forgive me, there are responses in this forum, however, I still didn't see a
response.

You could use an option button (aka radio button) and group them by utilizing
the group name. This way the user can only select one button within each
group.
