to remove check box
 

Click on it and then hit the delete key.

You may need to use:
View|Toolbars|Control Toolbox
then click the Design mode icon on that toolbar
then click on the checkbox and hit the delete key

Then click once more on the design mode icon to turn that off.

If the worksheet is protected, you may need to unprotect it, too.
