Duplicate A5 worksheet with header & footer onto an A4 original
 

I have a 40 odd page list of document titles and associated data that have to
be output to create an A5 booklet. The printers would like to use A4
originals of each sheet and then guillotine the sheets to make two booklets.
The booklets are in landscape with a different header & footer on the first
page.
Each page is number automatically.

This is my first outing in this forum and I am looking for any guidance,
help, tips or pointers to information that I can find.
