Need excel tool that helps assess, score, rank Supplier Risk
 

I"m looking for an excel template that I can use to assess different kinds of
Supply Chain risk with Suppliers. Ideally, the tool would calculate a risk
score that I can use to rank Supplier risk and prioritize contingency plans.
